There are many references to some pointe shoe fitting "form" Ms. DeVor has for people who are having difficulty finding shoes that fit. I can't find this anywhere on the website! How can I access form?

Go to the "Pointe Shoes Topics" forum. Look at the 'pinned' topics at the top. Click on the last pinned topic, "Pointe Shoes Suggestion Form Link." The link to the form is in there.
