In my experience,  these late substitutions are either typed on a slip and inserted into the program or announced prior to the show to the audience...
Tonight, so and so will be performing the role of milkmaid. It all depends how far in advance you know there's a change.

Thanks- program has not yet gone to printer so question is, traditionally is the dancer still listed as US in program even though they are dancing in place of an injured dancer? Thanks in advance!

If the program has not yet gone to the printer and the understudy (D. Dancer) has moved up to the role because the previous dancer in the role (B. Ballerina) is injured, then the program should just list D. Dancer in the role.

The real purpose is to give credit.  So proper is either a swap if there truly is time.  But if not an insert or announcement at the show.  "Tonight the role of XYZ will be danced by ABC".  Whether it has been printed, could be anything from taken to the printer and not yet printed to not yet typed up and sent in.  So it all depends on when the show is and what "not printed" means.
